If your department is unable to submit data by the required due date, request either a temporary or permanent extension of the due date for your department's monthly run data submission. A DOH Administrator will accept or deny the request so you can address submissions as appropriate. Regardless of an extension, you must submit data for your department at some point in time. Acceptance of a request is not an exemption from submission.
To add an Exception request
From any page, go to Requests: Exception. The Exception Requests page opens.
Click Add Exception Request. The Add Exception Request window opens.
Enter information in the fields.
Exception Request Fields
Email of the individual making the Exception request.
Phone number of the individual making the Exception request.
The month for which you are making the Exception request.
The desired extension date.
The reason for the Exception request; the reason the extension is needed.
Any additional comments regarding the Exception request.
